Registered Dietitian

(Integrated Position)
Temporary Part-Time (0.75FTE)

The Great Northern Family Health Team and the Haileybury Family Health Team are collaborative inter-disciplinary teams of physicians and primary health care providers with a vision to enhance primary health care, improve patient access and integrate a collaborative care model for patients. The Great Northern Family Health team and the Haileybury Family Health Team are committed to providing safe and accessible patient-centered care.

We are currently seeking a motivated and organized Registered Dietitian to join our team to fulfill a temporary part-time (0.75 FTE) vacancy that works jointly with the Great Northern Family Health Team (0.5 FTE) and Haileybury Family Health Team (0.25 FTE).


Reporting to the Executive Directors, you will provide dietitian services within a multidisciplinary team of colleagues. Key responsibilities include:

  • Perform client intakes, screening and therapeutic and preventative nutrition assessments.

  • Provide comprehensive nutrition education and counseling activities including health promotion and education.

  • Design and implement goal-oriented nutritional care plans.

  • Create, maintain and secure personal health information/documentation related to the client’s treatment

  • Work collaboratively in a team environment as well as to function independently

Qualifications:

  • Baccalaureate of Science in Nutrition or equivalent from a recognized university;

  • Currently registered and in good standing with the with the College of Dietitians;

  • Experience in the area of health promotion, chronic disease management and prevention and adult education considered an asset;

  • Certified Diabetes Educator considered an asset;

  • Familiarity working with Electronic Medical Records;

  • Strong clinical thinking and problem-solving skills;

  • Must have an excellent work record, including attendance.

  • Demonstrated effective interpersonal and communication skills with the ability to communicate effectively with patients and families, colleagues and other health care professionals.

  • Must support and contribute to a culture of safety.

  • Oral proficiency in French at an advanced or superior level considered an asset.
